Receiving Services

How do I start receiving services after I have had my appointment?

1. Each semester students must pick up ACCOMMODATION FORMS at the DSS office. (The amount of requested forms should reflect the number of classes you are enrolled in for the current semester. Each instructor and teaching assistant (TA) will need a copy in order to allow you your accomodations.) This process is repeated EACH SEMESTER!!.
   
   
   
   
   
2. Once you have received the ACCOMMODATION FORM, give each of your professors a copy as proof of your eligibility for accommodations.
