Documentation Forms

The type of documentation necessary will vary depending on the type of disability and accommodations being requested. Typically, individuals with medical or psychiatric conditions will be asked to provide documentation from a physician. Individuals with learning disabilities, attention deficit disorder or attention deficit hyperactivity disorder will be asked to provide a full psychoeducational test report.

DSS Students' Forms

  You can use either of the two methods to complete your request:

  #1: DSS Test Request Form / a.k.a. Pink Sheet. - To connect to online form request

  OR

  #2: DSS Test Request Form / a.k.a. Pink Sheet and Test Authorization Form - To print hard copies and submit by hand

  • Use the documents above to complete the Test Request Form (a.k.a. The Pink Sheet).
  • Print Test Authorization form to give to your instructor, and the sheet must accompany the exam delivered to the DSS Testing Office.
  • E-mail completed Test Request Form (Pink Sheet) to DSSTest@umd.edu, or hand deliver to 4120 Susquehanna Hall. DSS Testing Office hours Mon. thru Fri., 9AM to 4PM. Phone: 301.314.7217.
