How do I sign up for my exams?

  1. Schedule to take your test at DSS from 9am-4pm Monday through Friday. These are the only times you can schedule at DSS. (EXCEPT during final exam week). If you have conflicts with your classes in planning time for your exam, please discuss this matter with the Testing Coordinator. REMEMBER: Professors have the final say on the alternate test date!

  2. FILL OUT a Test Request Form (Pink Sheet) for each exam.
    REMEMBER: PINK SHEETS must be submitted to DSS at least 5 WORKING DAYS before the exam.

  3. If you DON'T hand in the PINK SHEET within the 5 DAY POLICY, you will be put on the BEEN WARNED list once a semester. After that first time, you must speak with your professor about giving you the accommodation.

    • What is the pink sheet?
    • The Pink sheet/the Test Request Form is your reservation appointment for the exam at DSS. The information you provide about your accommodations allows DSS to reserve a space for you to arrange for a computer, private space, reader or a scribe, etc. You still need to give your professor the white Test Authorization Form to take a test at DSS, and that must come over with your exam. (DSS will not administer exam without the TAF attached).

      Remember:

      • You, the student, are responsible for filling out the agreed upon day and time of the test.
      • Figure out the testing time allowance based on your extended time accomodations.
      • You are responsible for checking the appropriate accommodations listed on your Accomodations Letter (bring your File Copy with you to the Testing Office; your information willbe verified by the Testing Office Coordinators).

How does my instructor accommodate my exams at the DSS office?

  1. Fill out the top part of the white Test Authorization Form with the requested day and time of the upcoming exam.

  2. Take the Test Authorization Form (TAF) to your instructor five (5) days before the exam. This will help remind your instructor to deliver the exam to the DSS Testing Office on time.

    Exams can be delivered to DSS Testing office by one of four options:

    • By fax (301.405.0813)
    • Hand deliver (4120 Susquehanna Hall)
    • Email (DSSTest@umd.edu)
    • Have student bring the test to DSS on the date of the exam (in SEALED and secured envelope).

    Note: Your test WILL NOT be administered without the TEST AUTHORIZATION FORM attached to the exam!

What do I do if a student brings me an accommodation form?

  1. The Accomodation Letter indicates the specific accomodations the student is eligibile to receive. Please keep the copy for your records and reference.

  2. The Accommodation Letter should be given to you during the beginning of the semester or anytime during the semester IF a student is a new registrant with DSS.

  3. Once you have received the accommodation form, DSS recommends both student and instructor (and TAs) discuss the accommodations.
    • This is very important because the student may have scheduling conflicts that need to be mutually agreed upon. For example, a student may have back to back classes on the day the test is scheduled and may need to reschedule for another time.

  4. University of Maryland, College Park offers instructors the choice of providing equivalent accommodations in the classroom OR sending exams to DSS. Either way, DSS recommends instructor and student discuss these choices.

    • If questions are related to the student's academic accommodations, please call the main office 301.314.7682 to speak with the student's counselor.

What do I do if a student gives me a white Test Authorization Form?

  1. It is the student's responsibility to bring you the TEST AUTHORIZATION FORM five (5) days before scheduled date of an exam, if you both have agreed exam will be administered at DSS Testing.

    • Check to make sure the date and time are correctly filled out.
    • Make sure you fill in the bottom half of the form (indicating specific instructions; exam return address, your contact information during the exam and exam time).
    • Students are required to take exams at the SAME TIME as the rest of class.
    • Students can only change exam times, with written permission from instructor (written permission must come over with the exam).
    • DSS cannot administer the test without this form attached with exam.

  2. The completed TAF form and exam can be delivered to the DSS Testing Office by three options:
    • By Fax: 301.314.9478
    • Hand delivered: 4120 Susquehanna Hall (office hours Monday-Friday, 9am-4pm) .
    • E-mailed: DSSTest@umd.edu
    • All options must include the Test Authorization Form.
